A second look at cases called “impossible”

Uzm. Dt. Rouzbeh Fazeli is a co-founder of İzmir Dent and has worked at our Karabağlar clinic as a periodontology specialist and implantologist since 2021. His focus is the situations where standard implant protocols fall short: a resorbed jawbone, a dropped sinus in the upper back jaw, advanced gum disease, and cases where every tooth has been lost.

His clinical approach rests on two principles. The first is to adapt the treatment to the biology the patient already has, rather than forcing the anatomy to fit a protocol. The second is to avoid major surgery when the same result can be reached with less of it. So in place of plans that begin with extensive bone grafting, suitable cases are treated with methods that use the existing bone more efficiently — shorter healing, and markedly less pain afterwards.

In which cases should you seek a consultation?

• You have been told elsewhere that there is not enough bone for an implant, or that a large bone graft must come first
• Bone height in the upper back jaw has decreased and the sinus has dropped
• Every tooth has been lost, or the remaining teeth cannot be saved — a fixed full-arch solution is needed
• Signs of advanced gum disease: receding gums, bleeding, bone loss
• Inflammation or bone loss has developed around implants placed earlier
• Treatment has been postponed because of a fear of surgery

Methods used

   Closed (internal) sinus lift

   Bone height is gained through the implant socket itself, without the open sinus operation.

2. 02

   Short and narrow implants

   In suitable cases, implant treatment without bone grafting or augmentation.

3. 03

   Flapless, computer-guided implant surgery

   Placement through a digitally planned guide — no incision, no stitches.

4. 04

   Fixed full-arch restoration (All-on-4 / All-on-6)

   Implants placed and temporary fixed teeth fitted in the same session.
